If the lights went out because some bored operator was playing solitaire on his monitor, or the curret worm took down a MS computer.
Once I worked in a fair size facility that was machining items on large CNC centers. The Owner made a surprise inspection late at night after attending a party and found the unattended machines working with the operators all in a distant room used for programming playing a computer game.

I think it depends on who is to blame. If it is First Energy, I doubt we'll ever know the whole truth. I suspect human error. Even if the "alarms" failed, isn't there supposed to be someone monitoring the system? It was an adventure, that's for sure. For the most part, I think people handled it wonderfully. No looting (except in Canada). The majority of people on the road actually did the right thing by treating traffic lights as four-way stops. It was impressive.
