I was just wondering how many sellers use a fixed-price in their shipping and handling?
How many sell different items, all with different shipping and handling, but the shipping is matched to the item (such as you sell 20 different items, but they are broken into 4 categories with set prices for each)?

I have 4 tiers of S/H ($1.00, $2.50, $3.00, and $5.00) for 4 categories of products. Additionally, I provide combined S/H which varies by product line.

I use fixed prices, with the dollar amount stated in my listings. The fixed price varies with the weight of the book, i.e. up to one pound is X, one to two pounds is Y, two to three pounds is Z, etc. At each level I offer the choice of priority or media mail. And, of course, I combine shipping on multiple purchases, so two two-pound books are treated as one four-pound book.
Then there are corresponding fixed prices for both surface & air mail to various international destinations.
Right now, that means 6 possible fixed prices for each book, depending on shipping method & destination country. (I'm probably going to break up the international shipping a bit more, increasing the number to ten per book.)
Since most books are six pounds or less, as a practical matter, I have six different weight-based classes of fixed prices, each with six (soon to be ten) options, for a total of 36 (or 60) different prices.

Since I have a regular product line........Everything I sell is fixed priced and any auction site I sell it on I use the buy now options if the site has it.
I have 4 levels of shipping costs
$1.00, Small items
$2.00, Small/medium items under 1lb
$3.50 1 lb priority
$3.95 2 lb priority
I don't sell unique collectible items so I don't need to figure out shipping on an item by item basis.
A rarely deal with insurance unless it is a large order so I don't even bother recomending it to customers.
I don't charge handling fees basically just postage.

I use fix shipping in my auctions also. I sell dvds and offer customers a choice of either of two rates.
Priority $4 for the first two dvds, add $.50 for each additional (delivery time usually 2-3 days) or
Media Mail $2.25 for the firs two dvds, add $.50 for each additional (delivery time usually 7-14 days)
I include DC on all packages and it is included in the above rates.
