Item is in excellent condition.   Plays Great!!  Disc has a couple very small scratches.  Gameplay is not affected.  Protector plastic has some some punctures on back, middle bottom.  Plastic is a little rippled on front middle bottom.

Protector, artwork insert/game instructions and game disc included.

Game Disc(s) will be in paper dvd sleeve and inside of protector to prevent scratching while being shipped.
