This is the standard pandant shade itself, St. Agnes.  You have to go online and purchase the cord and ceiling pendant.  They are approx 16.00 or so.  I have wire and pendants for all of my other lamps auctioning.

This lamp, St. Agnes, is a faint green base with circle design in olive green and black design.
