This was purchased by gym owner 3 years ago.  Taken well care of.  #343, 34 bulb, 3 phase.  Facial tanners and fans.  3 Phase Commercial.

Taken apart for moving to the warehouse where it is being stored.  You can see photo.  There are two marks on the top in black.  See photos.  May be able to be cleaned off.  I did not have anything to try it with.
