I'm pretty sure that the person who told me Tom was out for the week having shoulder surgery WAS Tom.

'Cause Tom then sent me a very formal letter on his corporate letterhead, thanking me for the opportunity to buy the book for $8.99 and giving me instructions on two different mailing addresses, one for USPS and the other for UPS.

I think Stone is right - If I had cashed the check, he would have demanded the book (claiming that $8.99 constituted payment in full because the 3-day notice didn't include the $2), and there would have been some sort of threat of a complaint to eBay or negative feedback.

PS ...The envelope with the $2 that was supposedly mailed last Wednesday still hasn't shown up.

well,may be he is planning to resell this rare book at cost of 8.99 and not a penny more.
