From what I have found this is a haviland limoges dish. It is marked haviland and limoges, so that part was easy. I found a similar item on Ruby Lane http://www.rubylane.com/shops/alou955/item/772

Is Anchor Blank the name of the pattern? I couldn't tell what that was referring to.

Can anyone tell me what kind of dish this is? There is a ventilation hole in the lid. It stands about 6" tall and the base is 5" across.

It looks like a covered sugar. The lid with the ventilation hole could have gone on a teapot.

How big is it? Anchor blank might be a blank produced by Anchor that people used to decorate and paint themselves. It was very popular years back as a hobby.

Anchor blank might also be the blank they use for other dish sets. They produce the blank then they decorate it with the pattern of the dishes they use it with. Yes and china painting was also popular and I suppose many painters used that to make their pieces.

I doubt it is a covered sugar because sugar bowls wouldn't have a hole as you don't want moisture to get into the sugar. It is also big for a sugar. It's for something that is hot as to let the steam out.

It's an Escalvier Soup Tureen, very common in the Pa Dutch areas around Lancaster,PA!

Gut man!

I am betting the top has been switched and it came from a tea pot.
