This has happened quite often, so I thought I'd ask. Here's an example of an order number 560313440. I can't leave feedback for them - it just doesn't go through. I can go directly through ebay and leave the feedback, but not through the PSM. It's all an ebay purchase - no store items or other auction sites. Happens with about 1 in 5 orders.

It doesn't say anything. I click on the leave feedback button as usual and the screen disappears and comes back like it normally does, but the #7 (feedback box) isn't checked, so I check the buyers feedback and sure enough it didn't leave anything.

I am trying it from the post sale page - not sure though what the fullfillment checklist is - maybe I'm doing it from there. I'm doing it from the post sale section, where it has the area to choose custom or "already done" feedback choices. The only time I run into problems is with combined orders - and then it's only 1 out of 5 times.
If you there and select the first feedback button (the one with the check marks off that you already left feedback for this item) you should be able to choose the feedback you want to send.
